What if my X-rays are normal but I still hurt?

Short answer

Normal X-rays rule out most fractures and gross bone abnormalities but do not evaluate soft tissues, ligaments, discs, or nerves — where many crash injuries actually occur.

Deeper explanation

X-rays are excellent at showing bone. They are not designed to show muscle, ligament, disc, or nerve injury.

Many crash-related injuries involve soft tissues and the nervous system. A normal X-ray does not mean nothing is wrong — it means that specific set of findings was not present on that specific study.

When symptoms persist, more targeted evaluation — including detailed examination and, when clinically indicated, MRI or specialist referral — may be appropriate.

  • Progressive weakness, numbness, or loss of function
  • Bowel or bladder changes
  • Severe unrelenting pain

What a proper evaluation should include

  • Detailed orthopedic and neurologic examination
  • Functional assessment
  • Advanced imaging or specialist referral when clinically indicated

Documentation considerations

The record should reflect that imaging was performed, what it did and did not show, and what other objective findings are present on examination.
